We need this information to ensure it is safe for us to carry out the treatments you require. Depending on the service(s) you require we may need further information about your medical history. You do not have to share your personal information with us, but if you do not comply we will not be able to carry out the treatment. We also need contact details to provide confirmation of appointments and to send relevant information ie: pre/post treatment care.
